We would like to
take this opportunity to welcome you to our office and introduce you to our office policies, as follows:

Financial Responsibility
Payment is expected in full at the time of service. If using insurance, the patient is
expected to pay any co-pay, co-insurance, any unmet deductible or patient responsibility and we will gladly file the claim.
We accept cash, checks, or credit/debit cards for charges over $25.00. As a courtesy to the patient, we will file claims
for out-of -network benefits, however, the patient will be expected to pay when services are rendered.
• Insurance
Authorizations
Patients are responsible to obtain the initial authorization for any provider they choose to see.
Many insurance companies require pre-authorization in order to provide coverage. Please check with your insurance company
and we will be happy to follow up when visit authorizations need to be extended. We also need to be notified if there is any
change in insurance coverage. As for prescription coverage, some companies require pre-authorization or an override that
will handled by the pharmacy or doctor.
• Cancellation Policy
Since we do not double book appointment
times, our office policy requires a 2-business-day notice to cancel an appointment without charge. If a patient does not show
up for a scheduled appointment, the patient will be billed for an administrative fee (insurance cannot cover any service
not delivered). If emergencies arise, they will certainly be taken into consideration.
• Prescription Refills
Appointments are carefully planned so that patients will not run out of medication unexpectedly, therefore refill
requests are denied without an appointment. Medications need to be monitored closely and consistency of care is very important
to us.
• Emergency Treatment
If a registered patient needs emergency care, the doctor, doctor-on-call,
or nurse practitioner can always be reached by pager, through our office phone number. If the patient is a danger to himself/herself
or others, the patient needs to go to an emergency room in addition to notifying the doctor. This office does not provide
inpatient care.
